Learner Objectives

This session will help participants…

  1. differentiate roles and responsibilities of transition team members.
  2. formulate a generic transition plan based on goal and thematic skill needs.
  3. assemble transition goals that reflect student interests and strengths while accounting for where a student requires support.


For most students, school is not the end goal. Most are interested in higher education, employment, or independent living. Educational services, thus, should be viewed as a pathway for students to achieve their postsecondary goals. This presentation will delve deeply into IDEA (2004) transition planning requirements, review roles for each member of the transition team, and work to create a road map to guide educators, students, and families through the process for exiting schools.
